Big plans on the
Grand Concourse
This is a phenomenal opportunity for me to be
part of the Community Health Partnership. The CHP program, It's in Our Hands, has
resources that will allow me to educate the community on prevention and management
of chronic diseases and motivate the community toward taking greater control of
their health. The Hispanic community in the Bronx is lost in today's fragmented
healthcare system. I, too, was lost in the same fragmented system years ago, and
it took a major toll upon me and my family. Once I learned what to do to take control
of my life, I learned to truly live it. I want others to have the same opportunity
as I did, and I can do this via the CHP program.
