Our Mission
The Community Health Partnership at sanofi-aventis is working hard to guide patients
through a healthcare system they may find unapproachable and to motivate them to
take action. It's in Our Hands is the program that breaks down these barriers, opening
up local residents to new opportunities.
The Healthcare Challenges in Our Communities
Getting access to good healthcare is a given for some, while others face challenges
that prohibit it. Many African Americans and Hispanics are facing certain cultural
and social barriers that prevent them from getting the proper healthcare they need.
Bridging that gap will create awareness of existing community resources and services,
encouraging them to seek appropriate care for themselves and their family.
How We Can Help
Community Health Partnerships exist in six different communities with high concentrations
of African American or Hispanic patients. Liaisons in each city work closely with
the local community healthcare resources, while raising awareness among patients,
providers, and the Community Health Action Team. This team provides community health guidance and
recommendations for local events, speaker presentations, city-specific healthcare
resources and teaching tools. Together these efforts will ideally drive underserved
patients and caregivers to connect with these resources and create their own healthier
tomorrow.
