continues to experience disparities in healthcare. As in many other cities, local strategies to improve the health of
the community may often revolve around primary prevention and/or in-stage treatment
of the condition. The Community Health Partnership approaches this problem at all
levels and empowers the community, by linking all the healthcare providers, educators,
and prevention initiatives for the first time.
